Before You Remove That Age Spot, Rule Out the One That Isn't
Most flat brown spots on sun-exposed skin are harmless solar lentigines, but a small subset are early melanomas that look almost identical. Here is how clinicians tell them apart, and why that matters before any laser, peel, or cream touches your skin.

The typical age spot, known clinically as a solar lentigo, is a benign patch of pigment that forms after years of ultraviolet exposure. Chronic UV damage triggers a lasting increase in melanin production and a modest rise in the number of melanocytes, the pigment-producing cells, along the base of the epidermis. The result is a flat, tan to dark brown spot on the face, hands, shoulders, or forearms. It is cosmetically annoying but medically harmless.
The problem is that one particular skin cancer, lentigo maligna, is essentially a slow-growing melanoma that begins in the same sun-damaged skin and can look strikingly similar in its early years. It also appears as a flat brown patch on the face or other chronically exposed areas, often in people over 50. Studies of pigmented facial lesions consistently show that even experienced clinicians misjudge a meaningful fraction of them on naked-eye inspection alone. That is why the standard of care before any destructive treatment, whether laser, cryotherapy, chemical peel, or prescription lightener, is a proper diagnostic look first.
Why treating first is a genuine risk, not just a technicality. Lasers and intense pulsed light devices work on age spots by delivering energy that is absorbed by melanin, shattering pigment so the body can clear it. Cryotherapy freezes and destroys pigment-laden cells. Both approaches can visibly erase an early lentigo maligna without touching the malignant cells that remain scattered in the deeper epidermis and hair follicles. The lesion fades, the patient is reassured, and the cancer continues to grow quietly, sometimes for years, until it recurs in a more advanced and harder to treat form. Case reports in the dermatology literature describe exactly this sequence. Removing the visible pigment removes the warning sign, not the disease.
What clinicians actually look for. The classic screening framework is the ABCDE checklist, adapted here for flat pigmented spots:
Asymmetry: a benign lentigo is usually fairly uniform, while one half of a suspicious lesion often looks different from the other.
Border: solar lentigines can have irregular, moth-eaten edges too, so border alone is not decisive, but sharply notched or smudged margins raise concern.
Color: multiple shades within one spot, especially very dark brown, black, gray, or blue tones mixed with tan, is one of the strongest warning signs. A benign age spot tends to be one relatively even color.
Diameter: lentigo maligna often exceeds 6 millimeters, though so do many harmless lentigines, which limits this criterion on its own.
Evolution: this is the most useful signal in practice. Benign age spots are stable. A flat brown patch that is clearly enlarging, darkening unevenly, or developing new colors over months deserves evaluation, full stop.
Beyond the naked eye, dermatologists use dermoscopy, a handheld magnifier with polarized light that reveals pigment patterns beneath the surface. Benign lentigines typically show a fine, uniform network or a fingerprint-like pattern. Lentigo maligna tends to show asymmetric pigmentation around hair follicles, gray dots, and rhomboid or angulated structures. Dermoscopy substantially improves diagnostic accuracy compared with visual inspection alone. When the picture remains ambiguous, a small biopsy settles the question with histology, the microscopic examination of the tissue itself.
Who should be most careful. Risk concentrates in the same population that seeks age spot removal most often: people over 50 with fair skin, a history of heavy cumulative sun exposure, and visible photodamage such as many lentigines, deep wrinkling, or a history of actinic keratoses. Spots on the face, ears, and scalp warrant extra caution because that is where lentigo maligna most commonly arises. A prior history of skin cancer in you or a first-degree relative also shifts the calculus toward a professional check before cosmetic treatment.
Practical guidance before booking a removal. First, if a spot is new, changing, itching, bleeding, or simply looks different from your other spots, the so-called ugly duckling, have it examined medically before pursuing cosmetic options. Second, when you do seek removal, favor providers who examine lesions with dermoscopy or who are trained to recognize lesions that need biopsy rather than treatment. Reputable practices routinely decline to laser a spot they cannot confidently call benign, and that refusal is a good sign, not an obstacle. Third, understand that a spot fading after treatment is not proof it was harmless, so continue to watch treated areas and report any recurrence of pigment.
None of this should cause alarm about the average sun spot. The overwhelming majority of flat brown patches in older adults are exactly what they appear to be, and modern removal methods handle them well. The point is sequencing: diagnosis first, destruction second. A five-minute look with the right tools protects you from the rare but serious scenario where a cosmetic fix hides a cancer, and it costs you nothing but a short delay.
